Do you need a visa?
Yes. Belize's own nationality table lists India as visa-required. The table is on the Ministry of Immigration website.
A visa allows you to enter Belize and remain for a maximum of 30 days from the date of entry. The Ministry also says that any visit longer than 30, 60, 90 or 180 days can need a visa and the matching extensions, so plan a longer trip with the Immigration Department.

Exemption with a US, Canadian or Schengen visa
Belize exempts visits of up to 30 days from the entry visa if you hold a valid multiple-entry visa or permanent residence card from the United States, a valid multiple-entry visa or permanent residence card from Canada, or a valid multiple-entry Schengen visa. The table says multiple entry, so a single-entry visa is not the same thing.
Carry the visa or card with your passport, and ask the Belizean mission if you are unsure which of your documents qualify.

Documents
The visa requirements page asks for the completed application form, a passport valid for more than 6 months, and a flight itinerary that shows your arrival in Belize and your return to your country of origin.
If someone in Belize sponsors you, you also need their letter of financial support signed by a Justice of the Peace, their identification, proof of kinship, an invitation letter, and employment letters and six months of bank records for both of you. If you sponsor yourself, you need your own employment letter from the last six months and six months of bank records.

Where to apply and what it costs
Apply at your nearest Belizean diplomatic or consular mission, and submit the originals and copies of every document. Documents in languages other than English must be translated and signed by the proper authority.
The fee table, effective from 1 May 2020 and quoted in Belize dollars, lists a single-entry visa valid for 6 months for nationals of India, Nepal, Thailand, Burma, Bangladesh, Pakistan and Sri Lanka at BZ$750, and a multiple-entry visa valid for 12 months at BZ$1,000. The fees do not apply to permanent residents of the United States or holders of a US visitor visa. Check the current fee with the mission before you pay.

Staying longer than 30 days
The Visitor's Permit Extension adds 30 days and costs BZ$200 for all nationals. You request it at an Immigration office. The holder may not work, and anyone who needs a visa after a set number of extended days must get that visa before an extension is issued.
